It is recommended to show up 15 minutes early to complete your registration paperwork. If you prefer to do the paperwork ahead of time, visit our Online Forms Page, print the forms, fill them out and bring them with you to your appointment.
If you have records from a previous veterinarian please have them faxed to (952) 920-1229 or e-mailed to info@edinapethospital.com ahead of time if possible. If not, please bring them with you to your appointment. This will assist us in determining which services are necessary for your pet.
We recommend writing down any questions or concerns you have ahead of time so we can fully address all issues.
